Home Health & Palliative Services Mercy's Home Health program has a team of trained specialists available to assist patients and their families with palliative care services in their own home. Our professional, certified care teams work closely with the patient's doctor to ensure a continuum of care, as well as maintain an appropriate level of independence for the individual.
Hospice services range from skilled nursing in the comfort of the patient's home to pain management, caregiver support and more. The Mercy Hospice Team - comprised in part of registered nurses, clinical social workers, chaplains, home health aides and volunteers - works closely with the patient's doctor to ensure the patient's needs are met, as well as the concerns of the family, during what can be a very difficult time.

Advance Healthcare Directive (AHCD) An Advance Healthcare Directive allows you to provide direction about your care even if you cannot speak for yourself at the time. It also allows you to state your wishes regarding the withholding of life-sustaining treatment and pain control, and allows you to appoint the person of your choice to direct this care. All information you provide is helpful to your doctor, the nursing professionals caring for you, and the family or friends who may be called upon if and when you cannot speak for yourself.
